Ibrutinib in combination with obinutuzumab for the treatment of chronic lymphocytic leukaemia/small lymphocytic lymphoma in elderly – first line

The combination of ibrutinib (oral capsules) and obinutuzumab (intravenous injection) is being developed as a new treatment option for CLL/SLL in elderly patients. Both drugs act in different unique ways to improve the body’s natural defence to fight the cancer cells and their combined effect may significantly reduce symptoms of the disease and increase survival.

Lenalidomide (Revlimid) + rituximab for relapsed/refractory follicular lymphoma and marginal zone lymphoma

The drug combination of lenalidomide (oral capsules) and rituximab (by intravenous injection) has the potential to be an effective treatment for relapsed or refractory follicular and marginal zone lymphoma. The drug combination is being developed for patients that are eligible for rituximab monotherapy or for patients that need an alternative to standard chemotherapy regimens, where rituximab alone would be considered sub-optimal.

criticism of limitations in minimally invasive techniques regarding lymphadenectomy has been shown not to be evidenced. We describe the techniques of mediastinal lymphadenectomies during the uniportal VATS (uniVATS) approach with video attachments.

Measuring impact of chemotherapy upon AMH and fertility in breast cancer and lymphoma patients relative to normal controls- 5-year multicentre study

Reproductive age women with malignancy have lower AMH than controls even before starting chemotherapy. Our study confirmed strong correlation between pre-chemotherapy AMH and post-chemotherapy AMH (measured at 1 and 5-year) in young women diagnosed with breast cancer.

Low/undetectable serum AMH concentration 12 months after starting chemotherapy does not exclude future pregnancy. There is an unmet need for fertility consultation and preservation in young women diagnosed with cancer.

Obinutuzumab with bendamustine for treating follicular lymphoma refractory to rituximab [TA472[

New Evidence-based recommendations on obinutuzumab (Gazyvaro) with bendamustine for adults with follicular lymphoma that did not respond or progressed after treatment with rituximab.

Obinutuzumab in combination with bendamustine followed by obinutuzumab maintenance is recommended for use within the Cancer Drugs Fund as an option for treating adults with follicular lymphoma that did not respond or progressed during or up to 6 months after treatment with rituximab or a rituximab-containing regimen, only if the conditions in the managed access agreement for obinutuzumab are followed.

Liposuction for chronic lymphoedema Current evidence on the safety and efficacy of liposuction for chronic lymphoedema is adequate to support the use of this procedure provided that standard arrangements are in place for clinical governance, consent and audit.
Patient selection should only be done by a multidisciplinary team as part of a lymphoedema service.

Nivolumab for treating relapsed or refractory classical Hodgkin lymphoma [TA462]

New NICE Technology Appraisal Guidance on using nivolumab (Opdivo) for treating relapsed or refractory classical Hodgkin lymphoma in adults.

Nivolumab is recommended, within its marketing authorisation, as an option for treating relapsed or refractory classical Hodgkin lymphoma in adults after autologous stem cell transplant and treatment with brentuximab vedotin, when the company provides nivolumab with the discount agreed in the patient access scheme.

L-Dex U400 for lymphoedema after breast cancer treatment

New Medtech innovation briefing from NICE on using the L-Dex U400 for lymphoedema after breast cancer treatment.

The innovative aspects are that it uses bioimpedance spectroscopy to measure fluid status, aiming to detect lymphoedema earlier.

The intended place in therapy would be in diagnosing or monitoring lymphoedema related to breast cancer treatment. It could be used as an alternative to current detection or measurement techniques, such as simple tape measurement of limb circumference.

According to specialists involved in this evidence summary, infliximab may be an option for some patients with severe, refractory extrapulmonary sarcoidosis (particularly cutaneous or neurological sarcoidosis); for example, those affected by disabling or disfiguring disease, or whose life expectancy is likely to be reduced.

Implementation of ABS guidelines would have resulted in 16.6% of good prognosis cancers to be undertreated whilst 71.4% of bad prognosis cancers would be over-treated with unnecessary axillary surgery and its resultant morbidity.

Axilla management of patients with pre-operatively diagnosed lymph node metastasis currently leads to overtreatment in 31.3%. There is scope to identify patients with a low nodal burden to select them for axillary staging and possibly avoid ALNC.

PB.9 The use of nomograms to predict additional lymph node metastasis after Sentinel lymph node biopsy: Can they reliably identify those needing no further axillary treatment?

Overtreatment with ALNC is 75.8% in patients with sentinel node macrometastasis in 1-2 nodes. MSKCC nomogram had the best utility to identify this group with AUC 0.729. MDA2 and Helsinki nomograms also had good clinical use in deciding which patients should receive axillary clearance.

Pre-treatment MRI features can serve as reliable imaging biomarkers which can be used to predict disease response to NAC. The accuracy of post-NAC MRI varies with tumour biology and imaging features, and this should be considered in decisions about surgical approach.

PB.39 Outcome of assessment of patient reported symptoms during screening mammograms - Single NHS BSP unit's experience

This study shows the positive predictive value (PPV) of patients who reported symptoms turning into a breast cancer is low. Therefore, an alternate pathway of managing these patients should be investigated.

Synchronous Upper and Lower Gastrointestinal Mucosa-Associated Lymphoid Tissue Lymphomas

Mucosa-associated lymphoid tissue lymphoma (MALToma) is a subtype of B-cell non-Hodgkin’s lymphoma, comprising ∼17% of all gastrointestinal (GI) tract lymphomas. It is associated with chronic inflammation and autoimmunity, for example Helicobacter pylori gastritis and Sjogren’s syndrome, respectively. Approximately 50% of GI MALTomas occur in the stomach, with small bowel and colonic lesions being less frequent. Synchronous upper and lower GI MALTomas occur rarely, with few cases reported. We present the case of a 73-year-old patient who presented with change in bowel habit and was found to have synchronous multifocal upper and lower GI MALTomas, which did not respond to H. pylori cure or to rituximab therapy, but did respond to a combination of surgery and chemotherapy with rituximab and bendamustine.

Non-Hodgkin’s lymphoma: diagnosis and management

New NICE guideline covering diagnosing and managing non-Hodgkin's lymphoma in people aged 16 years and over. It aims to improve care for people with non-Hodgkin's lymphoma by promoting the best tests for diagnosis and staging and the most effective treatments for 6 of the subtypes. Tests and treatments covered include excision biopsy, radiotherapy, immunochemotherapy and stem cell transplantation.
