Sustainability and Transformation Fund (STF) and financial control totals: updated guidance
The 2017/18 and 2018/19 guidance has been revised to take account of the updated eligibility criteria for the 30% performance element of STF from quarter 2 onwards.
The payment of the whole 30% of these funds at Q2 and Q3 will be based on a combination of achieving the A&E 4 hour performance standard and also the required milestones on primary care front door streaming. The changes are detailed in section 3.5 of the revised guidance.

Why should STPs worry about primary care?
Sustainability and transformation partnerships (STPs) have a tough job to do – focused on making the books balance, hamstrung by provider deficits, trying to make an unsustainable system sustainable.
Yet the majority of patient contacts and opportunity for upstream intervention is in primary care, most obviously general practice but also community pharmacies, dentists and optometrists. Accountable care organisations (ACOs) explained
Briefing from the King's Fund about accountable care organisations (ACOs). NHS England has recently outlined ambitions for sustainability and transformation partnerships (STPs) to evolve into ‘accountable care systems’ (ACSs), and proposed that these ACSs might become ACOs but only after ‘several years’. Eight areas of England have now been identified to lead their development. They comprise three core elements.
First, they involve a provider or, more usually, an alliance of providers that collaborate to meet the needs of a defined population. Second, these providers take responsibility for a budget allocated by a commissioner or alliance of commissioners to deliver a range of services to that population. And third, ACOs work under a contract that specifies the outcomes and other objectives they are required to achieve within the given budget, often extending over a number of years.

Integrating care: contracting for accountable models NHS England
NHS England has published information on contractual arrangements for accountable care organisations.
This package clarifies how accountable models may be contracted for, how local areas would approach procurement questions and the application of the new Integrated Support and Assurance Process, run by NHSE and NHSI in relation to the award of all novel or complex procurements by commissioners. It sets out the funding model including the details of integrated budget approaches, a new Improvement Payment Scheme to replace current incentive schemes and gain/loss sharing across the local health economy to align incentives. Finally, the package describes implications for the commissioning system of new models of whole population provision.

End of Life Care data to inform Sustainability and Transformation Partnerships
Public Health England in partnership with NHS and Care Quality Commission (CQC) present key data about end of life care to assist Sustainability and Transformation Partnerships (STPs) as they develop their plans for services.
PHE are publishing the data supplied to this project as a spreadsheet (End of Life Care STP Data Tool) together with a metadata guide (End of Life Care STP Data Tool MetaGuide).
The spreadsheet includes the most recently available data 25 indicators presented at STP and CCG level, and includes the number of deaths, the age, cause and place of deaths, data describing hospital admissions near the end of life together with information on the number of care home beds and use of palliative care registers by GP practices.

Patients to benefit from £325 million investment in NHS transformation projects
Fifteen sustainability and transformation partnerships (STPs) will share in £325m of capital investment to take forward transformation projects. The funding, announced by health secretary Jeremy Hunt and NHS England chief Simon Stevens on 19 July, has been targeted at the “strongest and most advanced schemes in STPs.”
